COMMUNITY RESIDENCES are provided to individuals who need 24 hour supports. Support services are tailored to individuals' needs in order to maximize their potential for independence and provide opportunities to make meaningful community connections. Services include:

• Intake and needs assessment
• Transition planning
• Individualized Service Plan
• Skills development and training
• Case management and support
• Recreation and community access
