 ADULT FAMILY CARE (AFC) provides financial and clinical assistance to qualifying family members or caregivers to assist them in caring for their loved ones at home. We support, but do not limit our outreach to, individuals with developmental disabilities and seniors. The AFC Team provides:

  • Nursing support and oversight
  • Care management and support
  • Assistance accessing community services
  • Training and networking
  • Financial compensation

LETTER FROM MIEKE MONEN, DIRECTOR OF RESIDENTIAL SERVICES: Financial and Clinical Assistance for Your Child or Loved One Who is Currently Living at Home with You

Dear Family Member:

Given the latest news from the state regarding DESE/DDS funding for families with special needs members, we want to remind you that Minute Man Arc has been certified to provide financial and clinical assistance to qualifying family members to assist them in caring for their loved ones at home. This program is called Adult Family Care (AFC).

Adult Family Care is a Mass Health funded program that provides 24-hour services in a personalized home setting.   This program allows people with chronic health care needs to maintain their dignity and pursue typical life routines with individualized assistance from a family or non-family caregiver. Eligible recipients of this program must be at least 16 years of age, receive Mass Health Insurance (either Standard or Common Health) and receive daily assistance with completing tasks from a caregiver who lives in the same home.  AFC recipients must require assistance in at least one of the following areas:  Mobility in or outside of their home, bathing, dressing, toileting, eating or transferring.  A registered nurse and care manager will conduct an assessment to determine eligibility.

AFC is a new and innovative option for families. Reimbursement is determined by your family member’s level of need. Reimbursement can range from $8,000 to about $18,000 tax exempt, per year. The AFC multi disciplinary team consisting of a nurse, care manager and program director will provide consistent, professional support services to family members through nursing oversight and individualized, thoughtful care management.
